This form must be completed and either sent to the Secretary or brought to Conference. NOTE: Your delegate cannot be seated without this form.

This form is for pastors to state their intentions for the coming church year. This form is submitted to the Stationing Committee to assist them in assigning pastors for the coming year.

This form is for all members of the Conference to submit their address information to the secretary for the Minutes. Please be sure to complete this form and turn it in at Conference.

Each ordained Elder is required to complete this form and turn it in to the Ministerial Education and Relations Committee each year.
